KELLER BOLLYWOOD DANCE STUDIO (KBDS) - MEMBERSHIP AGREEMENT

Lessons/Pricing
Group classes are currently $120 month for (4) classes.
Private lessons are $140 for (4) 30 minute lessons, $200 for (4) 45 minute lessons, or $240 for (4) 60 minute lessons
On Months with 5 lesson slots, one week will be an (off week) for students and teachers, unless otherwise offered for an additional fee.
Tuition/Fees
$99 A one time registration fee per family will be charged upon enrollment or re-enrollment.
Tuition is due on the last lesson of EACH MONTH - no exceptions- and will be run via our auto-pay system.
If a payment is returned for NSF or not paid on time for any reason, there will be a $30 reprocessing fee.
Delinquent payment may forfeit lesson times slotted, and subject your student to dismissal.
In order for a student to participate in any of the programs offered at KBDS all paperwork and fees must be up to date. It is imperative that you keep your e-mail address current throughout the year.
**(The enrollment fee is waived if the semester is paid in advance)**
Attendance
We allow no more than two (2) makeup lessons per semester. 48 HOURS NOTICE MUST BE GIVEN TO THE INSTRUCTOR to qualify for a makeup lesson. Our teachers are paid by the lesson period that you reserve whether you use that spot or not. Makeup lesson times are subject to teacher's availability.
Group lesson students are allowed to attend another class day or time if they can't make their scheduled day but must give the instructor direct notice.
There are NO refunds credits, or pro-rated tuition for missed lessons.
Recitals and Concerts
Specific recital dates and times will be announced as soon as they are finalised. Students will be invited to concerts and recitals, student participation depends on their level of preparation and commitment.
Students may be asked to volunteer as stage hands, media group, or various other assignments to make the concerts and recitals a success.
The average recital and concert fee is $30 per student unless otherwise indicated. This covers any additional practice and rehearsal times.
There are no classes on concert days & the concerts and recital days count as a lesson time
Conduct - KBDS and Pandemonium Music reserve the right to ask anyone to leave if they are not acting in the best interest of the academy, its staff, or students. If a student is disruptive, inattentive, or behaves in a way not conducive to a classroom environment, they will be asked to leave for that lesson with no makeup. If the behavior occurs (2) times they will be removed from the program.
If a class enrolment is too small, we reserve the right to cancel or combine that class with another appropriate class. Also, if during a group class there is only 1 student present, it will be treated as private lesson & only meet for 30 minutes. If a private lesson spot becomes available, we may adjust your lesson time to prevent gaps in our teachers schedules.
Insurance - Keller Bollywood Dance and Pandemonium Music Studios do not carry medical insurance for its students. It is required that all music students be covered by their own family's insurance policies and if injury occurs, it is understood that the student's own policy is your only source of reimbursement.
INJURY - Dance is not without its risks and this or any other exercise program may result in injury.
You should always consult your physician or other healthcare provider before starting a new exercise program
As with any exercise program, if at any point during your workout you begin to feel faint, dizzy or have physical discomfort, you should stop immediately and consult a medical professional.
The student/guardian acknowledges that any injuries, losses or damages they might incur on the premises are their responsibility; the studio, its instructors, trainers, partners and employees are not liable for any losses, injuries or damages that may occur on or near the premises.
Lessons Discount - We loves to see our students able to play a variety of instruments. Therefore, we offer $10 off each additional class enrolled in, every month. We also give a courtesy discount of $10 off a month for each family member enrolled
Referral Credit- Your biggest compliment to us is your referrals. We offer $25 off YOUR tuition for each family that signs up (after their second month of enrolment).
Cancellation - Please give us the courtesy of 30 days for lesson cancellation so that we have the opportunity to fill your lesson time slot with a new student.
Withdrawals - Any student who drops out of the program MUST FILL OUT OUR LESSON CANCELLATION FORM AND SUBMIT TO OUR OFFICE 30 DAYS FROM THE CANCELLATION DATE. This will stop the billing process. If the customer docs not give written notice, monthly tuition will still be processed through auto-pay for the next month's classes and they will be financially responsible for the amount. IF YOU ATTEND ANY PORTION OF THE MONTH, YOU ARE RESPONSIBLE FOR THE FULL MONTHLY TUITION.
